Novo Nordisk's Wegovy pill headlines mask complicated competitive reality

Novo Nordisk's (NYSE:NVO) Wegovy pill is running three times ahead of rival Zepbound at the equivalent stage of launch, according to pharmacy data, a headline that implies dominant market positioning in oral GLP-1 therapeutics. But the data supporting that claim comes with substantial qualifications that tell a different story.

The tracker shows Wegovy Pill at 162,000 prescriptions in its 23rd week, a modest 1 per cent weekly increase.

Yet the comparison to Zepbound explicitly excludes direct-to-patient cash-pay channels, a significant distribution route for weight-loss drugs that many patients self-fund. The headline metric, therefore, captures an incomplete market picture.

More telling is demand dynamics within the Wegovy pill itself. The lowest-dose formulation declined 4 per cent weekly, suggesting potential price sensitivity or patient migration to alternatives at higher price points. That weakness occurred despite a post-holiday rebound week, indicating underlying softness rather than seasonal volatility.

Lilly's new Foundayo, launched only weeks ago, reached 21,648 prescriptions in its ninth full week versus Wegovy's 81,000 at the same milestone. The raw numbers favour Novo, but Citi flags a critical caveat: pharmacy capture rates vary considerably, and it remains far too early to draw reliable conclusions.

This mirrors the wealth exodus narrative discussed by UBS economist Paul Donovan: a compelling headline preceded by detailed caveats that substantially complicate the story. Markets hear the headline; careful readers absorb the qualifications.

The GLP-1 competitive race is real and significant for investors. But headline prescription counts that exclude major distribution channels and rest on incomplete early-launch data risk misleading as much as they inform. The question is whether the market will do the harder work of reading the fine print
